IMPORTANT NOTICE

Monday 30th October 2017

With effect from 12 noon

Due to the high volume of patients wishing to register at this surgery, we are temporarily capping our list in order that we can continue to provide a safe and timely service to existing patients.

If you are part of the Cornwallis Group of practices, including Essenden Road, who have received a letter saying you are out of area, please call the helpline number below as some of these letters have been sent out incorrectly.

01424 735666

Email: cornwallisplazaenquiries@nhs.net

If you are a new patient to the area wishing to register at a practice, please contact the allocation line on 01424 735652.

We apologise for any inconvenience caused.

When you attend for your next appointment why not use our new checking in service? As you walk into the waiting room you will see the new automatic check-in ahead of you, to the left. Just touch the screen and follow the instructions. To use it, all you need to know is your date of birth! It really is that simple!

PLEASE NOTE: GP appointments are 10 minutes in duration.

From the 1st September 2017 you will no longer be able to order repeat prescriptions via the surgery email address.

We offer Online Patient Access which will enable you to book appointments and order repeat prescriptions.

To register, please visit the surgery to complete a registration form, bringing with you two forms of identification, one clearly stating your address.

Click on our new 'Dementia Support and Information' page (on the right hand side of the home page) for lots of help and advice on dealing with dementia.

Home Visits

Doctor's bagWhilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception between 08:30 - 10:00.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
