IMPORTANT NOTICE

Monday 30th October 2017

With effect from 12 noon

Due to the high volume of patients wishing to register at this surgery, we are temporarily capping our list in order that we can continue to provide a safe and timely service to existing patients.

If you are part of the Cornwallis Group of practices, including Essenden Road, who have received a letter saying you are out of area, please call the helpline number below as some of these letters have been sent out incorrectly.

01424 735666

Email: cornwallisplazaenquiries@nhs.net

If you are a new patient to the area wishing to register at a practice, please contact the allocation line on 01424 735652.

We apologise for any inconvenience caused.

When you attend for your next appointment why not use our new checking in service? As you walk into the waiting room you will see the new automatic check-in ahead of you, to the left. Just touch the screen and follow the instructions. To use it, all you need to know is your date of birth! It really is that simple!

PLEASE NOTE: GP appointments are 10 minutes in duration.

From the 1st September 2017 you will no longer be able to order repeat prescriptions via the surgery email address.

We offer Online Patient Access which will enable you to book appointments and order repeat prescriptions.

To register, please visit the surgery to complete a registration form, bringing with you two forms of identification, one clearly stating your address.

Click on our new 'Dementia Support and Information' page (on the right hand side of the home page) for lots of help and advice on dealing with dementia.

Appointmentscalendar

Due to the increase in demand for appointments, the doctors have requested that receptionists enquire as to the nature of your appointment.

This is to direct you to the correct Healthcare Professional, and also to avoid the booking of things that can be done outside an appointment, such as repeat prescriptions and sick certificates.

To see a doctor please telephone the appointments line on 01424 432925 or come into reception. The telephone line is open between 08:45 - 12:00 and 14:00 - 18:00 Monday to Friday.

We will aim to give you a 10 minute appointment on the same day, although you may  book up to 4 weeks ahead if that would be more convenient for you.

We kindly ask that all patients try to stick to our one problem per consultation policy.

Please Note: GP appointments should not be requested for medical certificates or repeat prescription requests

You can now book and cancel appointments on-line. To access this service you will need to call in at the surgery personally in order to sign up and receive your registration details. Once you have registered, follow this link to access the website. https://patient.emisaccess.co.uk

Telephone Advice

If you have a problem which you wish to discuss by telephone please call between 10:00 - 16:30 and give the receptionist the details. Your call will then be dealt with by an appropriate member of the team.

Practice Nurses

In a number of cases it might be worth considering an appointment with a practice nurse rather than a doctor. Practice nurses are qualified to deal with many ailments and you may be seen more quickly.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.
